These resources provide specific information to help General Practitioners (GPs) and other health care professionals when contacting Armadale Health Service (AHS).

Updating your Details

To check or update your practice details or enrol for electronic or faxed discharge summaries and outpatient letters, please email AKG.webPASSupport@health.wa.gov.au.

Please provide the following information:

  • Your Practice name, address, phone, fax, email)
  • A list of all current GPs at your practice including provider numbers
  • Your practice EDI code for secure electronic transmission (obtained from your secure messaging provider).

Once updated, your new practice contact details will be available to other public hospitals in WA, excluding some public-private hospitals which use different systems.

You are responsible for ensuring your details are correct and ensuring fax machines or computer systems provide adequate privacy after transmission.

 

Discharge summaries

These will be uploaded to the patient's My Health Recordunless they request otherwise.

Electronic summaries are also sent to a patient’s nominated GP using secure electronic messaging or by fax.

Hard copies are only posted to recipients who haven’t been sent a copy by secure messaging or fax. Patients will be given a hard copy if the summary has been finalised before they are discharged.

Emergency Summaries

When patients attend the Emergency Department (ED), a Communik8 summary will be faxed to the patients nominated GP. This only provides limited information.

If urgent information is required about a patient, contact the On Duty ED Doctor on 9291 2176.

 

Outpatient Letters

Armadale and Kalamunda Hospitals send outpatient clinic letters to GPs and referrers via secure message, fax or (if this fails) by mail.

  • Most outpatient letters, pathology and imaging results are uploaded to the patient’s My Health Record.
  • If you receive an outpatient letter for a patient who does not attend your practice, phone the Medical Typing Unit on 9391 1011 or email AKG.Transcriptions@health.wa.gov.au (include patient name, Date of Birth, medical record number, speciality and date of letter).
  • If your contact details or list of doctors needs updating, email AKG.webPASSupport@health.wa.gov.au

 

 

Outpatient appointments are made after a valid and complete referral is received from a General Practitioner (GP), other health service or community health care provider.           

Immediate outpatient referrals

    Armadale Health Service operates three Immediate access clinics for patients who require care within seven days.

    These are:

    • Rapid Access foot line (Podiatry)
    • Orthopaedic Trauma Clinic (after acute management has been provided)
    • Early Pregnancy Assessment Clinic (Gynaecology)

     

    For patients who require immediate referral to an acute service, contact the On Call Specialist via switchboard on 9391 2000.

    This ensures the specialist team is immediately aware of the referral, allowing them to agree on a plan and make arrangements as soon as possible.

    These arrangements include interim treatment recommendations and further investigations required prior to AHS review. Please note who you spoke to and any instructions you are given.  

     

     

    Once there is agreement that the patient requires an appointment within seven days, the next step is to email the patient’s referral to:

    Ensure the referral is marked IMMEDIATE and include the name of the registrar or consultant spoken with at the hospital. Include all essential referral information, investigations and clinical photos.

     

    Routine outpatient referrals

    All non-urgent GP referrals to doctor-led outpatient clinics should be sent to the Central Referral Service (external site) and provide current (within the past four weeks) clinical handover.

    Referrals are accepted on the GP letterhead generated from your practice software or using an online referral form.

    The following patient information is required:

    • Date of birth.
    • Address
    • Contact phone numbers
    • Medicare number (including expiry date)
    • Patient's maiden name
    • Language requirements (interpreter)
    • Current medication and allergy information
    • Weight (particularly if over 120 kilograms)
    • Copies of relevant investigations and reports
    • GP contact details.

     

    Please check the Referral Access Criteria (RAC) for the speciality you are requesting at Central Referral Service Referral Access Criteria

    Referrals must meet the RAC to be accepted. Referrals which do not contain sufficient information to allow accurate triaging will be returned to the referrer.

    Patients with cognitive or memory impairments

    In these cases, agreements are often made to have appointment details mailed to their carer or relative, allowing them to arrange transport and ensure they attend. If this applies, please indicate in the ‘special needs’ section of the referral form.

    Priority

    Immediate referrals are those which are discussed with a speciality, accepted to be seen within seven days and sent direct to site.

    All other referrals are clinically triaged as:

    Urgent     Priority 1   Up to 30 days 
    Semi urgent Priority 2 31 to 90 days
    Routine Priority 3 91 to 365 days

     

    Waiting times for some specialities fall outside of these triage categories

    Note: Referrals for Nursing, Midwife, Allied Health and Mental Health do not go to CRS.

    Obstetric referrals:

    Complete the Antenatal Care and Delivery Referral Form PDF or Antenatal care and delivery referral form RTF and email to akg.referrals@health.wa.gov.au.

    Mental health referrals:

    Complete the Adult mental health referral or the Older Adult Mental Health GP referral form and email to akg.referrals@health.wa.gov.au.

    Our mental health outpatient centres do not offer programs for:

    • primary marriage counselling
    • intellectual Impairment services
    • drug, alcohol and substance abuse.

    Allied Health Referrals

    Complete the AHS Rehabilitation and Allied Health referral form or in rich text format and email to akg.referrals@health.wa.gov.au.

Please check your patient’s My Health Record before requesting from medical records.

A faxed or emailed request on the GP practice letterhead/email is required to authenticate request.

This must include:

    • full patient name
    • date of birth
    • medical record number (if known)
    • information requirements

Urgent Admissions

Priority of admission is given to patients with the greatest medical needs.

Patients will only be admitted under the care of doctors with admitting rights to Armadale Health Service.

Factors considered in determining priorities for admission include:

  • Medical need
  • Other patients also requiring admission in the immediate future
  • Staffing levels

  • Availability of beds and other resources.

     

All admission queries should be discussed with the relevant on call specialist via the switchboard (Ph: 9391 2000).  Patients will usually be directed to attend the Emergency Department.

Emergency admissions

Contact the ED Duty Officer on 9391 2176 (if liaising with the On Call Specialist is not suitable).

Requests for Infusions

Direct all queries to the relevant On Call Specialist on 9391 2000.
