Medical Specialty Referrals

Referrals for Medical led Specialty outpatient services are to be sent via the Central Referral Service (CRS):

Please check the Referral Access Criteria (RAC) for the speciality you are requesting at Central Referral Service guide for referrers (external link). Referrals to specialties with RACs must meet the RAC to be accepted. Referrals which do not contain sufficient information to allow accurate triaging of the priority of the referral will be returned to the referrer.

Nursing, Midwifery, Allied Health and Day Hospice Referrals

To be sent to the Armadale Kalamunda Referral Office. This office can be contacted if any queries on referral processes, referral status or direct referrals (verbally accepted by specialist).

For escalation of a referral please forward an updated referral to the Central Referral Service (above) with new information and reason for escalation. Sending a duplicate referral is not sufficient.

Immediate Referrals (for urgent care within 7 days)

Immediate referrals should be directed to the Specialist on-Call at Armadale Health Service (08) 9391 2000 and must be accepted verbally prior to direct referral.

These can then be emailed to AKG Referral office if approved by the Specialist for URGENT care. Please provide Approvers Name.

If not approved as urgent please forward referral as normal or send patient to ED if required.
